Overview

Clinical Evaluation Services provides comprehensive clinical evaluations to individuals with diverse needs. Our mission is to provide clients with thorough and well-written reports, in a timely and affordable manner. We are happy to work with your attorney when needed, in order to provide the most valuable and comprehensive document possible.

We specialize in reports designed to assist individuals going through the Immigration process, individuals in need of an Emotional Support Animal prescription, and Elderly Home Evaluation reports intended to assist families with an elderly family member living independently.

All evaluations are confidential and are conducted by a licensed clinical social worker. Our services are individually crafted and based on specific needs, and [excepting the Elderly Home Evaluation] may be completed electronically, or via phone.  For a more personal experience, in-person consultations are available (depending on your location) for all evaluations. All of our clinical reports are client-specific, address specific concerns, and may be updated or modified as needed. When necessary, our clients are provided with additional support and referrals to address any concerns noted in the evaluation process.

Emotional Support Animal

Anyone who has had a beloved pet understands the comfort and calming effect that animals can have on a person. For a person who suffers with debilitating anxiety, depression, PTSD, or other mental health concerns, the benefits can be extremely positive.

Having your pet designated as an Emotional Support Animal not only will allow more housing options for those living in communities that exclude pets, but can also eliminate travel fees on many airlines.

 

Emotional support animal and fair housing

*The Fair Housing Amendments Act of 1988, Section 504 of the Rehabilitation Act of 1973, and Title II of the Americans with Disabilities Act protect the right of people with disabilities to keep emotional support animals, even when a landlord's policy explicitly prohibits pets. Because emotional support and service animals are not "pets," but rather are considered to be more like assistive aids such as wheelchairs, the law will generally require the landlord to make an exception to its "no pet" policy so that a tenant with a disability can fully use and enjoy his or her dwelling. In most housing complexes, so long as the tenant has a letter or prescription from an appropriate professional, such as a therapist or physician, and meets the definition of a person with a disability, he or she is entitled to a reasonable accommodation that would allow an emotional support animal in the apartment. 

The Air Carrier Access Act (49 U.S.C., 41705 and 14 C.F.R. 382) informs that certification [as an ESA] of your pet is not necessary or required and provides no additional rights. The only required document is a prescription letter, which is provided after a clinical evaluation is completed. The prescription letter is a certification by a Licensed Clinical Social Worker, or other mental health provider, identifying an emotional disability that requires an emotional support animal.

Pursuant to C.F.R. 164.506, airlines are only permitted to verify the authenticity of your prescription. All client information is personal and will not be released to anyone.

 

Most airlines now have multiple steps for carrying an emotional support animal. We will provide an PDF version of your letter for upload. See Delta’s page for an example.

FEES:

 

An Emotional Support Animal evaluation is a confidential questionnaire that provides the clinician with personal information needed to assess a level of disability. If a determination of need is made, a letter for both housing and air travel will be provided. Air travel letters / housing letters are $125.00.

Letters (prescriptions) are good for one year. Annual renewal letters are provided after a short evaluation and are available for $55.00.

 

ESA Terms and Conditions

__I acknowledge that completion of the Emotional Support Animal Evaluation does not guarantee that I will receive an ESA letter (prescription).  Thus, I acknowledge that the evaluation fee is non-refundable

__The information that I provide will be thoroughly evaluated by a Licensed Clinical Social Worker. All information provided is confidential, with exceptions in the case of threat of harm to self or others.

__In some cases, a phone consultation, at no additional charge, will be required to gather additional information, or to offer additional supportive services and/or referrals.

__I understand that if I have no known history of anxiety, depression, PTSD, trauma, or other mental health concerns, I am advised to contact the office to discuss if the Emotional Support Animal Evaluation is applicable to my situation.

Immigration Reports

A psychosocial evaluation is a thorough and comprehensive report that provides substantial details on all aspects of a client’s life. Client circumstances are extracted during an interview, and documented in a manner making them both accessible and comprehensible to the legal team.  Our skilled clinicians are able to provide a compelling analysis of the hardships faced by the client, providing invaluable data that will be used to complete the immigration application. The fee for this service is not only much less than attorney fees associated with gathering information, but also provides data that is often not otherwise discovered.

The initial meeting with the social worker is done either in person or on the phone. The confidential report is completed efficiently, and is tailor made to the specific needs of the case. The objective of the clinical immigration report is to both support and enhance the immigration case, while adding emotional and compelling data to support the attorney. The clinicians at Clinical Evaluation Services are advocates for those seeking legal immigration, and are both committed and honored to be providing assistance to those in need.  The most common types of cases in which a psychosocial evaluation is used include Extreme Hardship, Domestic Violence, and Political Asylum.

 

*We recognize current immigration policy is experiencing unprecedented changes. The clinicians at Clinical Evaluation Services will maintain communication with the client’s legal team to ensure that all needed details are included to support the legal analysis.

 

FEES:

Immigration psychosocial evaluations are detailed clinical documents that provide an expansive amount of information used by the immigration lawyer in support of their client. The initial interview in conducted in person or via phone, with a fee of $150.00 per hour. The report fee is $75.00 per hour. Most reports are completed in less than seven hours.

Immigration Report Terms and Conditions

__I understand that information provided in the Immigration report is designed to support the work of my immigration attorney. The Immigration Report is not a guarantee of a positive outcome, and therefore, the interview and report writing fee is non-refundable. I understand that every effort is made to complete the report in a timely manner.  Payment for the initial interview will be required at the initial meeting. The final payment will be due on receipt of the evaluation report.

Elderly Home Evaluation

Having an elderly family member living alone can cause challenges in any family. Elderly individuals striving to maintain independence may choose not to share the actual struggles that they are experiencing on a day-to-day basis. Often, this selfless act is done in order to keep their perceived difficulties to themselves, without burdening their adult children. The Clinical Social Worker acts as both an advocate and support person, providing an inside and objective analysis of the client’s living situation. The home visit is most often perceived as a pleasurable experience for the person living alone, and provides valuable information, resources and referrals to the family providing support for their elderly relative. The Home Evaluation will include a subjective and objective analysis, as well as an assessment, and plan.  All visits are conducted by a licensed clinical social worker and are available in the tri-county South Florida area (some restrictions apply).

 

 

FEES:

An Elderly Home Evaluation consists of a minimum of one in-home visit, with additional visits on an as-needed basis. A detailed report is provided as well as at least one phone consultation to the family. The fee for this service is $150.00 per in-home consultation hour. The report fee is $75.00 per hour.

Elderly Home Evaluation Terms and Conditions

__In some cases, a phone consultation, at no additional charge, will be required to gather additional information, or to offer additional supportive services and/or referrals.

__I acknowledge that a home evaluation assessment will include the emotional, physical, and psychosocial situation observed at the time of the home visit. Referrals and recommendations will be provided as needed. The home visit will be conducted by a Licensed Clinical Social Worker, and will be scheduled prior to the day of visit. The consultation fee is due prior to the home visit.  The written report fee will be required when the report is complete.

Victoria Schreiber MSW, LCSW has been writing clinical evaluations for individuals for over 20 years. A former school social worker, medical social worker, and childbirth educator, she is acutely aware of the psychosocial challenges facing today’s families. A dedicated and compassionate advocate for those in need, Victoria created Clinical Evaluation Services with the goal of providing personalized clinical reports to address the concerns she often observed in her practice.  She has dedicated herself to assisting immigrants, elderly individuals living alone, and people of all ages who are struggling with anxiety, depression, PTSD, or other mental health challenges. Through specialized client evaluations, Victoria is able to meet the specific needs of her clients in both an affordable, and efficient manner. Her attention to detail is surpassed only by the compassion she has for the clients she serves.

Victoria lives in Fort Lauderdale, Florida with her husband and has three grown children. She continues to provide clinical services to families of babies in a Neonatal Intensive Care Unit. She is an avid runner, knitter, beekeeper, lover of animals and all things outdoors. She holds both an undergraduate, and graduate degree in social work.
